The assessment process includes three phases: 1) data collection, 2) integrating results into a written report and 3) report release and review.

  • Data collection involves in-person testing, collateral interviews with caregivers, treatment team members and review of treatment records.
  • Integration of results begins once all data is collected and involves review of literature, consultation and report writing.
  • Once completed, the report is released and findings are reviewed with the client, caregivers and treatment team members. In the case of adolescent assessment, review of the results with the adolescent is provided when clinically indicated.

Fees

Fees vary based on testing needs specific to each client and an individual quote will be provided once information is gathered from an initial set-up call. Price is determined based on supplies and measures needed, predicted time to be spent in-person and collecting collateral information, testing location, timeline needs, etc. Lastly, a portion of all fees are donated to allow for Dr. Collett’s pro bono work and charitable donations.

  • Full payment is required prior to the onset of the assessment process.
  • Travel reimbursement is invoiced separately following in-person testing and payment is required before the completed report is released.
  • Dr. Collett is not in-network with any insurance companies and does not provide pre-authorization. However, upon request, a custom superbill can be provided to clients to be submitted to their insurance for potential reimbursement following the release of the report.
